Vincent, as many other painters of his time, did many different self-portraits. It was polite to send to your friends and family a self portrait that would show you how well you were doing, specially if you were living far from home. Van Gogh took this custom to a different level, painting himself exactly how the mirror would reflect him. When he had his ear cut off, he painted a self portrait with a bandage in his ear, looking not all that well.
